鞍结节脑膜瘤(TSMs)最佳的治疗方案是手术全切除肿瘤,且能最大限度地保护视觉功能。合理选择手术入路是TSMs成功、安全切除的关键。本文从TSMs手术的目的和适应证出发,就TSMs手术入路选择的标准和要求、手术入路与肿瘤全切除和术后视力改善的关系,以及经颅入路和经鼻蝶入路的适应证和优缺点进行总结分析,以期更精准地指导临床医生如何选择最佳的手术路径。

The optimal treatment for tuberculum sellae meningioma (TSMs) is to remove the tumor completely and maximize the protection of visual function. Reasonable selection of surgical approach is the key to successful and safe resection of TSMs. Based on the purpose and indications of TSMs, this review discusses the criteria and requirements for the selection of surgical approaches for TSMs, the relationship between surgical approaches and total tumor resection and postoperative vision improvement, and the adaptation of transcranial and transsphenoidal approaches, so as to guide clinicians to choose the best surgical path more accurately.
