目的

研究血清维生素D在儿童细菌性脑膜炎病程中的水平变化及其与患儿的临床关系。

方法

选择广东省廉江市人民医院儿科自2018年1月至2019年9月收治的43例细菌性脑膜炎患儿作为研究对象,液相色谱-串联质谱法检测患儿在第1、14、28天时血清维生素D水平,分析血清维生素D水平的变化特征。根据血清维生素D水平变化特征将患儿分为观察组和对照组,比较2组患儿在第1、14、28天时GCS评分、小儿危重症评分(PCIS)、脑脊液生化(白细胞、蛋白、葡萄糖)水平差异。

结果

29例患儿血清维生素D水平连续性下降,作为观察组;14例患儿血清维生素D水平变化趋势无规律或持续性升高,作为对照组。2组患儿维生素D水平差异具有统计学意义(t=5.600,P<0.05)。观察组患儿GCS评分、PCIS评分在第1天时与对照组比较,差异无统计学意义(P>0.05),在第14、28天时均小于对照组,差异具有统计学意义(P<0.05)。观察组患儿脑脊液白细胞数在第1天时与对照组比较,差异无统计学意义(P>0.05),在第14、28天时均高于对照组,差异具有统计学意义(P<0.05)。观察组患儿脑脊液蛋白量在第1、14天时与对照组比较,差异无统计学意义(P>0.05),在第28天时高于对照组,差异具有统计学意义(P<0.05)。观察组患儿脑脊液葡萄糖量在第1天与对照组比较,差异无统计学意义(P>0.05),在第14、28天时低于对照组,差异具有统计学意义(P<0.05)。观察组患儿出现并发症、需要手术治疗的例数均少于对照组,差异具有统计学意义(P<0.05)。

结论

部分细菌性脑膜炎患儿可出现血清维生素D水平的异常降低,临床治疗中较正常血清维生素D水平患儿更易出现并发症,需要手术治疗的概率更高,且患儿的康复较慢,应引起重视。

Objective

To investigate the level of serum vitamin D in the course of bacterial meningitis in children and its clinical relationship with children.

Methods

Forty-three cases of children with bacterial meningitis admitted to pediatrics department of Lianjiang People’s Hospital from January 2018 to September 2019 were selected as study subjects. The serum vitamin D levels of the children were detected by liquid chromatography-tandem mass spectrometry on day 1, 14 and 28, and the change characteristics of serum vitamin D levels were analyzed. Children were divided into observation group and control group according to the change characteristics of serum vitamin D level, and then the difference of GCS score, pediatric critical illness score (PCIS), cerebrospinal fluid (CSF) biochemistry (WBC, protein, glucose) between the two groups was compared on day 1, 14 and 28.

Results

Twenty-nine cases showed a continuous decrease in serum vitamin D level, serving as the observation group. Fourteen cases serum vitamin D level showed irregular or continued to increase, serving as the control group. There was a significant difference in vitamin D levels between the observation group and the control group (P<0.05). There was no significant difference in GCS score and PCIS score between the observation group and the control group on the first day (P>0.05), but it was smaller than that of the control group on day 14 and 28 (P<0.05). There was no significant difference between the observation group and the control group in the number of CSF white blood cells on the first day (P>0.05), but it was higher than the control group on day 14 and 28, with statistical difference (P<0.05). There was no significant difference in CSF protein between the observation group and the control group on day 1 and 14 (P>0.05), but it was higher than the control group on day 28 (P<0.05). There was no significant difference in CSF glucose between the observation group and the control group on day 1(P>0.05), but it was lower than that of the control group on day 14 and 28 (P<0.05). The number of complications and surgical treatment cases in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).

Conclusion

Some children with bacterial meningitis may have an abnormal decrease in serum vitamin D level, which is more likely to cause complications in clinical treatment than normal children with serum vitamin D level, and the probability of requiring surgical treatment is higher, and the recovery of children is slower, which should be paid attention to.
