颅底凹陷(BI)是颅颈交界区的一种骨性结构畸形,主要表现为齿状突异常突入至枕大孔,造成枕大孔区拥挤压迫,而产生相应延髓、上颈髓、后组颅神经等症状。影像学在BI诊断中的意义重大,临床上通常需要症状结合影像学共同诊断。但隐匿性或特殊类型(无明显失稳)的BI和该病对颅颈交界及上颈椎稳定性影响的判断,临床上尚存在争议。本文围绕BI的影像学传统诊断方式及最新进展进行总结,分析BI诊断中常用的传统影像学测量指标以及颅颈交界、上颈椎稳定性的判断标准,同时纳入近年来对BI诊断及稳定性评价提出的新影像学指标或标准,以期更好地指导临床实践中对BI的诊断、治疗决策以及预后评价。

Basilar invagination (BI) is a bony structural deformity in the cranio-cervical junction, which is characterized by abnormal protrusion of the dentate process to the foramen magnum, resulting crowding in the foramen magnum and symptoms of medulla oblongata, upper cervical spinal cord and posterior cranial nerve. Imaging plays an important role in BI diagnosis. And the diagnosis of BI usually requires symptoms combined with imaging. However, when it comes to the diagnosis of special type of BI which is latent or no obvious instability and the judgment of craniocervical junction and upper cervical stability, there are always controversies in clinical practice. This review intends to summarize the conventional imaging measurement stander and the latest imaging progress, and analyze the conventional imaging measurement stander that commonly practiced in diagnosis of BI as well as the judgment of craniocervical junction and upper cervical stability. Meanwhile, this review also summarizes some recent concepts of stability evaluation and imaging diagnosis of BI, for the sake of better guidance of the clinical practice of BI diagnosis, treatment decision-making and prognosis evaluation.
