目的

探讨丹参多酚酸联合尤瑞克林治疗急性分水岭脑梗死(ACWI)的临床疗效以及对患者认知功能、脑血流动力学和血清溶血磷脂酸(LPA)、氧化型低密度脂蛋白(ox-LDL)、基质金属蛋白酶-9(MMP-9)水平的影响。

方法

选取解放军联勤保障部队第九二八医院全科医学科自2019年1月至2021年6月收治的80例ACWI患者,使用随机数字表法分成观察组与对照组,每组40例。对照组给予尤瑞克林治疗,观察组在对照组基础上联合丹参多酚酸治疗。2组患者连续治疗3周,观察临床疗效,比较治疗前后蒙特利尔认知评估量表(MoCA)评分、美国国立卫生研究院卒中量表(NIHSS)评分、脑血流动力学参数[大脑前动脉(ACA)、大脑中动脉(MCA)及椎动脉(VA)的收缩期峰值流速(Vp)和阻力指数(RI)]及血清LPA、ox-LDL、MMP-9水平。统计2组患者不良反应的发生情况。

结果

观察组总有效率(92.50%)高于对照组(75.00%),差异具有统计学意义(P<0.05)。2组患者治疗后MoCA子项目评分及总分均显著高于治疗前,观察组改善更显著,差异具有统计学意义(P<0.05)。与治疗前相比,2组患者治疗后NIHSS评分和ACA、MCA及VA的RI均降低,ACA、MCA及VA的Vp均增高,且观察组NIHSS评分和ACA、MCA及VA的RI均低于对照组,ACA、MCA及VA的Vp均显著高于对照组,差异均具有统计学意义(P<0.05)。2组患者治疗后血清LPA、ox-LDL和MMP-9浓度均显著低于治疗前,且观察组低于对照组,差异均具有统计学意义(P<0.05)。2组患者的不良反应率比较,差异无统计学意义(P>0.05)。

结论

应用丹参多酚酸联合尤瑞克林治疗ACWI的整体疗效确切,能安全有效地减轻患者神经功能缺损及改善其认知功能,并能进一步改善脑血流动力学及下调血清LPA、ox-LDL和MMP-9的表达水平。

Objective

To investigate the clinical effect of salvianolic acid combined with urinary kallidinogenase on acute cerebral watershed infarction (ACWI) and its influence on cognitive function, cerebral hemodynamics and serum levels of lysophosphatidic acid (LPA), oxidized low density lipoprotein (ox-LDL), matrix metalloproteinases-9 (MMP-9).

Methods

Eighty patients with ACWI admitted to General Medicine Department of 928 Hospital of PLA Joint Logistic Support Force from January 2019 to June 2021 were randomly divided into observation group and control group (40 cases each). The control group was treated with urinary kallidinogenase, and the observation group was treated with salvianolic acid. All subjects were treated continuously for 3 weeks after the observation of the clinical efficacy of the two groups. The scores of montreal cognitive assessment (MoCA), national institute of health stroke scale (NIHSS), the peak systolic velocity (Vp) and resistance index (RI) of the anterior cerebral artery (ACA), middle cerebral artery (MCA) and vertebral artery (VA), and the levels of LPA, ox-MMP-9 were compared between the two groups before and after treatment. The adverse reactions of the two groups were also analyzed.

Results

The total effective rate of the observation group (92.50%) was higher than that of the control group (75.00%), and the difference was statistically significant (P<0.05). The MoCA subitem scores and total scores of the two groups after treatment were significantly higher than those before treatment, and the observation group showed greater improvement (P<0.05). Compared with before treatment, NIHSS score and RI of ACA, MCA and VA in the two groups after treatment were decreased, and Vp of ACA, MCA and VA were increased, and NIHSS score and RI of ACA, MCA and VA in the observation group were lower than those in the control group, and Vp of ACA, MCA and VA in the observation group were significantly higher than those in the control group (P<0.05). The serum concentrations of LPA, ox-LDL and MMP-9 in the two groups after treatment were significantly lower than those before treatment, and those in the observation group was lower than those in the control group (P<0.05). There was no significant difference in the adverse reaction rate between the observation group and the control group (P>0.05).

Conclusion

The overall effect of salvianolic acid combined with urinary kallidinogenase in the treatment of ACWI is definite, which can safely and effectively relieve the patients' neurological deficit and improve their cognitive function, it can further improve the expression level of LPA, ox-LDL and MMP-9 in the brain hemodynamics and down-regulate the blood.
