目的

探讨帕金森病(PD)患者的胱抑素C(CysC)、同型半胱氨酸(Hcy)水平及其对PD伴发脑白质病的评估价值。

方法

选择广西医科大学附属民族医院神经内科自2020年12月至2021年12月收治的124例PD患者,按照PD症状时期分为早期PD组(61例)、中晚期PD组(63例),采用头颅MRI检查PD患者中是否伴发脑白质病,并将患者分为伴发组和无伴发组。纳入同期于医院体检的115名健康志愿者为对照组。分别比较早期PD组、中晚期PD组和对照组、伴发组和无伴发组的血清CysC、Hcy水平,采用Logistic多元回归分析血清CysC、Hcy水平与PD患者伴发脑白质病变的关系,使用受试者工作特征曲线分析二者对伴发脑白质病变的评估价值。

结果

早期PD组、中晚期PD组患者血清CysC和Hcy水平均高于对照组,且中晚期PD组高于早期PD组,差异均有统计学意义(P<0.05)。124例PD患者中有35例伴发脑白质病变(伴发组),发病率为28.23%,余为无伴发组(89例)。伴发组患者血清CysC、Hcy水平明显高于无伴发组,差异有统计学意义(P<0.05)。年龄、PD病程、高血压病、糖尿病、血清CysC、Hcy均是PD伴发脑白质病变的独立影响因素(P<0.05)。血清CysC、Hcy水平评估伴发脑白质病变的Cut-off值分别为1.25 mg/L、21.02 μmol/L,联合评估PD对脑白质病的敏感性、AUC均高于单独指标评估,差异有统计学意义(P<0.05)。

结论

血清CysC和Hcy水平异常变化可作为临床评估PD患者发生脑白质病风险的重要参考指标,尤其是二者联合的评估价值更高。

Objective

To explore the levels of cystatin C (CysC) and homocysteine (Hcy) in patients with Parkinson's disease (PD) and their evaluation value for the development of white matter lesions in PD patients.

Methods

One hundred and twenty-four PD patients admitted to Neurology Department of the Affiliated Ethnic Hospital of Guangxi Medical University from December 2020 to December 2021 were selected and divided into early PD group (61 cases) and mid to late PD group (63 cases) according to the period of PD symptoms. One hundred and fifteen healthy volunteers who underwent physical examinations in the hospital during the same period were included as the control group. Brain MRI was used to examine whether PD patients were associated with white matter lesions, and the patients were divided into concomitant group and non concomitant group. The serum CysC and Hcy levels in early PD group, mid to late stage PD group, control group, concomitant group, and non concomitant group were compared. The relationship between serum CysC and Hcy levels and associated white matter lesions in PD patients were analyzed using logistic multiple regression, and the value of both two for associated cerebral white matter lesions was analyzed using receiver operating characteristic curves.

Results

The serum CysC and Hcy levels in the early PD group and the middle to late PD group were higher than those in the control group, and the middle to late PD group was higher than that in the early PD group, with statistical significance (P<0.05). Among 124 PD patients, 35 patients had white matter lesions (the concomitant group), the incidence rate was 28.23%, and the rest were non concomitant group (89 cases). The serum levels of CysC and Hcy in the concomitant group were significantly higher than those in the non concomitant group, with statistical significance (P<0.05). Age, course of PD, hypertension, diabetes, serum CysC and Hcy were all independent influencing factors for PD accompanied with white matter lesions (P<0.05). The cut-off values of assessing serum CysC and Hcy levels in patients with concomitant white matter lesions were 1.25 mg/L and 21.02 μmol/L, respectively. The sensitivity and AUC of PD for white matter lesions were higher in the combination evaluation than in the single indicator evaluation, and the difference was statistically significant (P<0.05).

Conclusion

Abnormal changes in serum CysC and Hcy levels can be used as important reference indicators for the clinical assessment of the risk of developing white matter lesions in PD patients, especially when combined, the evaluation value of the two is higher.
