目的

探讨基于想象疗法的闭环脑机接口训练对脑卒中后偏瘫患者上肢功能的影响。

方法

选取徐州医科大学附属徐州康复医院康复科自2020年2月至2021年5月收治的56例脑卒中后偏瘫患者为研究对象,按随机数字表法分为对照组和观察组,每组28例。对照组进行常规康复训练,观察组在对照组基础上进行基于想象疗法的闭环脑机接口训练。治疗前和治疗2、4周后均采用Fugl-Meyer运动功能量表上肢部分(FMA-UE)、卒中患者运动功能评估量表(MAS)、改良Barthel指数(MBI)、运动功能状态量表(MSS)评估2组患者的上肢运动功能和日常生活能力。

结果

2组患者的FMA-UE、MAS、MBI、MSS评分经重复测量方差分析差异均有统计学意义(P<0.05)。2组治疗2、4周后FMA-UE、MBI、MSS评分均逐次升高,MAS评分逐次降低,且观察组治疗2、4周后FMA-UE、MBI、MSS评分均高于对照组,MAS评分均低于对照组,差异具有统计学意义(P<0.05)。

结论

基于想象疗法的闭环脑机接口训练能显著改善脑卒中后偏瘫患者上肢运动能力和肌肉功能,提高日常活动能力。

Objective

To explore the effect of closed-loop brain computer interface training based on imagination therapy on upper limb function of patients with hemiplegia after stroke.

Methods

Fifty-six patients with post-stroke hemiplegia admitted to rehabilitation department of Xuzhou Rehabilitation Hospital from February 2020 to May 2021 were divided into 2 groups according to the random number table, 28 patients in each group. The control group received routine rehabilitation training, while the observation group received closed-loop brain computer interface training based on imagination therapy. The Fugl-Meyer assessment upper extreme (FMA-UE), motor assessment scale (MAS), modified Barthel index (MBI) and motor status scale (MSS) were used to evaluate the upper limb motor function and activities of daily living of the 2 groups before treatment, 2 and 4 weeks after treatment.

Results

The FMA-UE, MAS, MBI and MSS scores of the two groups were analyzed by repeated measurement of variance, and the differences were statistically significant (P<0.05). After 2 and 4 weeks of treatment, the scores of FMA-UE, MBI and MSS in the 2 groups increased step by step, and the score of MAS decreased step by step (P<0.05). After 2 and 4 weeks of treatment, the scores of FMA-UE, MBI and MSS in the observation group were higher than those in the control group (P<0.05), and the scores of MAS were lower than that in the control group (P<0.05).

Conclusion

The closed-loop brain computer interface training based on imagination therapy can significantly improve the upper limb motor ability and muscle function of patients with hemiplegia after stroke, and improve the ability of daily activities.
